    ​Wear shoulder pads:
    1. Hockey jersey size charts assume you are wearing shoulder pads.
    2. Put on your shoulder pads
    3. Wrap a tape measure around your entire body underneath your armpits. We want to capture the widest circumference of your chest.
    4. Find your measurement, (i.e 46″) look up the size on the size chart  (below).  That size is an Adult Medium.
    5. If you choose the size Medium, the jersey will fit but may be snug (see video).  To have a looser fitting jersey SIZE UP! to a Large.
    If you are registering more than one player (Maximum of 3 players), please complete the additional registrations forms below. If you have completed the registration process scroll down to agree to the release waiver and to make your payment

    40th Annual Sicamous Hockey School Release Form 2027
    In consideration of the acceptance as a student in the SICAMOUS HOCKEY SCHOOL
    program we, the undersigned parents/participant, release, remise and discharge the
    SICAMOUS HOCKEY SCHOOL from all claims, actions, causes of actions, damages
    and demands by the undersigned parents for loss or injury resulting directly or indirectly
    from the participation of our child in this program.
    The undersigned parents/participant covenant and agree to indemnify and save
    harmless SICAMOUS HOCKEY SCHOOL from all claims, actions, causes of action,
    damages and demands which may be brought by or on behalf of our child against
    SICAMOUS HOCKEY SCHOOL arising out of his/her participation in the program,
    including all costs, damages and expenses in defending any such claims or actions and
    against any loss arising there from.
    We acknowledge that the student cannot be accepted into this program but for our
    executing this release and indemnity.
    We have read the release and understand that it is a full final release of all claims for
    injuries and damages sustained in the SICAMOUS HOCKEY SCHOOL program and
    have read over the agreement to indemnify and understand the responsibilities we have
    assumed there under.

    Payment Options:
    Important!!  Sicamous Hockey school is offering three payment options. 
    ​
    1. etransfer: Once you have submitted this form you will be directed to the payment page. Choose the etransfer payment and complete the transaction as a zero balance payment. Once you have completed the zero balance etranser transaction complete the payment process by emailing your etransfer to [email protected]. Failure to complete the payment process will result in the failure of the application for the hockey school to be processed and your son/daughter will not be registered for the hockey school.
    2. Debit card payment:  Once you have submitted this form you will be directed to the payment page. Your payment options are to pay the $200 deposit or the full payment on your credit card.  Please make your selection and complete the payment process. Failure to complete the payment process will result in the failure of the application for the hockey school to be processed and your son/daughter will not be registered for the hockey school.
    3. Credit card payment:  Once you have submitted this form you will be directed to the payment page. Your payment options are to pay the $200 deposit or the full payment on your credit card.  Please make your selection and complete the payment process. Failure to complete the payment process will result in the failure of the application for the hockey school to be processed and your son/daughter will not be registered for the hockey school. 

    Optional Registration payment plan.
    We are pleased to be able to offer you the option of making payments instead of lump sum payments. The $200 deposit upon registering is mandatory to guarantee a spot. The payment plan options are:
    1. Pay the balance owing by making a etransfer  anytime for any amount to Sicamous Hockey School at [email protected].
    2. Pay your balance by preauthorized credit card payments. To arrange contact the Registrar at 250-836-4613 or [email protected]
    3. Provide post-dated cheques for your balance for dates that work for you.
    4. Make credit card payments at any time over the phone for any amount throughout the year by calling Terry at 250-836-4613.

    DEADLINE FOR FINAL PAYMENT IS JUNE 1, 2027. All those paid in full by June 1 will be entered for 1(ONE) FREE 2028 REGISTRATION.
    CANCELLATIONS MUST BE MADE BEFORE MAY 15, 2027 TO RECEIVE A FULL REFUND. CANCELLATIONS AFTER MAY 15, 2027 WILL BE REVIEWED ON AN INDIVIDUAL BASES FOR REFUNDS.
